What do I write after 'subscribed and sworn by' on a certificate?

I have a document which states, “subscribed and sworn to me by…” and I want to know what should go there, please.S.P., Indiana

The signer’s name goes in the blank space after ‘sworn and subscribed by’ in this statement. The words ‘subscribed and sworn to me by…’ indicate that you are performing the notarial act in Indiana known as a “verification on oath or affirmation.”
