Analysis

The Homeless Status Certification is a form provided by the Illinois Secretary of State Department of Driver Services. It is completed by a designated agent or agency to affirm that an individual is currently experiencing homelessness. Submission of the form allows individuals without a permanent address to certify their status and qualify for a free standard Illinois State ID Card.

Known as Form DSD A 230 and available at https://www.ilsos.gov/publications/pdf_publications/dsd_a230.pdf, the Homeless Status Certification must be subscribed and sworn to before a Notary Public.
